To use the printable OPEN RECORDS REQUEST FORM, please include all of the following information with your written request:
- Your name and mailing address;
- Your telephone number or e-mail address; and
- A detailed description of the Public Record you are seeking.
Arrangements can be made for documents to be viewed at no charge.
Please note that the County has transitioned to accepting the universal request form developed by the Office of Open Records in order to better assist individuals in filing requests for records.
